"Worthy the Lamb"

I bought a Bible at a thrift store recently and a slip of paper with an illustration was in it. The illustration also had this anonymous poem. There is no author listed and I don't have any idea of the publication this came from. The lines are as follows:

Worthy the Lamb enthroned on high
To be exalted thus!
Worthy the Lamb who died, we cry
For He was slain for us.
Jesus is worthy to receive
Honor and power divine:
And, blessings more than we can give,
Be Lord, for ever Thine.
